Pet Transportation Service Agreement and Liability Waiver

This Agreement is entered into between Pet Lifeline Chicago, Inc. ("Service Provider") and the undersigned Pet Owner ("Client"). By utilizing our transportation services, Client agrees to the following terms and conditions:

1. SERVICES PROVIDED: Service Provider agrees to transport Client's pet(s) to and from designated locations as requested. Service Provider will exercise reasonable care for the safety and well-being of the pet(s) during transit.

2. CLIENT OBLIGATIONS: Client represents that the pet is in good health, up-to-date on vaccinations, and does not possess a known history of aggression toward humans or other animals. Client must provide a secure crate or harness if required for safe transport.

3. EMERGENCY CARE: In the event of a medical emergency during transport, Service Provider is authorized to seek veterinary care at Client's expense. Service Provider will make reasonable efforts to contact Client immediately.

4. LIMITATION OF LIABILITY AND WAIVER: Client understands and acknowledges that pet transportation involves inherent risks. Client hereby waives any and all claims against Pet Lifeline Chicago, Inc., its directors, officers, employees, and volunteers for injury, illness, escape, or death of the pet(s) during the course of services, except in cases of gross negligence. Client agrees to indemnify and hold Service Provider harmless from any third-party claims arising from the pet's behavior.

5. CANCELLATION AND FEES: Cancellations must be made at least 24 hours in advance. Service Provider reserves the right to refuse service if the pet presents a safety risk to the driver or others.

6. GOVERNING LAW: This agreement shall be governed by the laws of the State of Illinois.
