I, the undersigned, hereby authorize Pet Lifeline Chicago, Inc. and its designated representatives to provide emergency transportation for my pet(s) to the nearest available veterinary facility or designated emergency animal hospital in the event of a medical emergency.

I understand that Pet Lifeline Chicago, Inc. will make every reasonable effort to contact me or my designated emergency contact using the information provided on file. However, in cases where immediate medical attention is required to save the pet's life or alleviate suffering, and I cannot be reached, I authorize the attending veterinarian to perform necessary diagnostic procedures, medical treatments, or surgical interventions as deemed essential by the veterinary staff.

I assume full financial responsibility for all costs incurred for such transportation and veterinary care, including but not limited to, emergency clinic fees, diagnostics, treatments, and medications. I agree to indemnify and hold harmless Pet Lifeline Chicago, Inc., its employees, and volunteers from any liability arising from such emergency assistance provided in good faith.

This authorization shall remain in effect from the date of signature below until revoked by me in writing and delivered to Pet Lifeline Chicago, Inc.
